The application for state office consists of five (5) documents.  The documents required are:

• A Statement of Experience and Intent for OSNA Office (online application)
• A Candidate Information sheet (online application)
  
• Proof of current student status in an Oregon nursing or pre-nursing program (upload letter from Dean, Faculty Advisor, or upload a copy of your winter term class registration)
  
• Proof of NSNA membership (upload scanned membership card)
  
• A letter of faculty recommendation (to be e-mailed directly to the NEC Director)
